1. Current Requirements for Certification in Schema Therapy (Individual)
2. Checklist of documentation (with PDF document fields) or Checklist of documentation* (with Word fields). Please email one of these forms along with your application to your regional coordinator. (In order to fill in and save the pdf forms you need a current and compatible PDF reader, for example Adobe Reader, google Chrome or Microsoft Edge.) 
3. Certification Application  (with PDF document fields) or Certification Application* (with Word fields) to submit for: Individual, Child-Adolescent, Couples, Group and Auxiliary Schema Therapist. (In order to fill in and save the pdf forms you need a current and compatible PDF reader, for example Adobe Reader, google Chrome or Microsoft Edge)
