    A new mode to help pathological worry

    The winner of the 2016 Young Investigator Award was announced at the Closing Ceremony of the INSPIRE 2016 ISST World Conference in Vienna: Robert Brockman. Additionally, three runners-up were announced: Harold DadomoDuygu Yakin, and Theano Tolgou.

    Robert Brockman discusses his winning topic, "Testing a schema mode model of pathological worry and generalized anxiety disorder," in an interview with Travis Atkinson. Learn how to frame worry in the schema therapy model, and how to use clinical interventions to effectively treat chronic worry.

    "Self-States: What They Are, What They Aren't"

    Watch the entire keynote address by Richard Chefetz 

    What are "self-states" and how are they related to modes? The latest and perhaps most useful iteration of a long line of references to compartmentalized and otherwise isolated aspects of the human mind, "self-states" are implicated when people think, speak, or act in ways that are at odds with how they usually are. For instance: 

    “I don’t know what got into me to say that! I’m so sorry. I couldn’t help it. I’m so ashamed.”

    This is a common exclamation we hear coming from our own mouths and those of our patients when self-states are activated. Self-states are especially relevant when working with eating disorders, sexual acting-out, all manner of addictive behaviors, post-traumatic disorders, complex PTSD, and the dissociative disorders. 

    Borrowing from Self Psychology, Object Relations, Relational Psychoanalysis, Attachment Theory, Affect Theory, and Traumatology, Richard Chefetz presents an integrated model for understanding self-state organization and activity on a subjective level that has immediate applicability to everyday clinical work, and can be used to expand our work with modes.

    The schema therapy mode model: scientific foundations and clinical practice

    What is it with modes? For many reasons, the schema therapy model that has become most prominent is based on the schema mode model. Perhaps this predominance stems from the schema mode model being easier for many patients to understand, and easier for therapists to apply. A key question in an evidence-based environment is, to what degree is the schema mode model scientifically supported?

    In his keynote address opening the INSPIRE 2016 ISST Vienna Conference, Arnoud Arntz discusses the scientific foundation of the schema mode model, as well as what we know from research about the mode model's helpfulness in clinical practice.

    2016 ISST Board Elections

    Nominations will take place in September, with online elections scheduled for October.

    GET INVOLVED: Every two years, elections take place to fill the positions on the ISST Executive Board. The ISST Election Committee, nominated by the current board, consists of John Gasiewski (Chair), and Erin Bulluss. They will be sending instructions over the next week outlining the process to nominate your favorite candidate to fill each board position.  

    Please watch for the election committee's email in your inbox, or view the details that will be posted on the ISST website. Details will include important dates of the nomination period, job descriptions of each board member, dates for online voting, a run-off election date (if necessary), and the date of the final announcement of the 2016 to 2018 ISST Board Members.

    "From what you see to what you need: towards an Interpersonal Schema Therapy" 

    Keynote by Eckhard Roediger 

    Modes are basically regarded as what we see. But behind the executed behavior, there is a layer of internal motivational processes. While more emotionally unstable patients express their mode activations quite directly, people with more prefrontal control might perceive modes as internal activations that are not necessarily acted out visibly. When we include "backstage" activations in interpersonal relationships within an extended schema-mode model, we understand mode clashes by referring to basic emotions caused by core needs that are unfulfilled. Guided by our Healthy Adult mode that expresses core needs in a balanced way, interpersonal conflicts can more effectively be resolved.

    Dear ISST Members,

    Scams are not new to the internet world. Thanks to feedback from many ISST members, we were able to identify a pattern of a recent scam that targeted select ISST members. After a thorough review, I want to reassure members that we have no evidence that a security breach of our media occurred. This was not a “hacking.” Our new website and listserv use the latest security features to protect your data. All of the information that the recent scammers used was publicly available on the internet.

    Our new website uses safety features to ensure that your email is not available to web browsers, but still allows searchers interested in contacting you to do so through the website, a significant upgrade from our past websites. With the help of many members, along with Chris Hayes, the ISST Secretary, we were able to identify a pattern of the scammers. To lessen the possibility of future scams, our ISST President, Eckhard Roediger, acted swiftly to redirect the old ISST website (www.ISSTOnline.com) to our new website (www.SchemaTherapySociety.org). However, scammers can still use multiple methods to "con" people, and ultimately it is up to every individual with a web presence to know the warning signs of scams, and to protect themselves from malicious cons.

    In the latest incident, the scammers used “Spear Phishing” with “soft targeting” methods to capture publicly available names of a few ISST members, including leaders of the organization, and to build “characters” around “stranded traveler” names to then email and, in some cases, even call members they selected to play out their scam. This is not the first time members have been targeted with similar scams, and we can expect more in the future. A quick Google search will show you that these “stranded traveler” scams have thrived since 2012, and are becoming even more common now (ransom scams are up 800% in 2016). 

    Scams can happen through email/text/phone/mail or any other method to contact you. Scammers use an individual's web presence against them. For instance, they can search social media and other easily accessible and publicly available media outlets to build a personality around the characters they have selected, and then to prey on members’ sympathy to “save” the “stranded traveler.”

    By having a web presence in any form, whether as a member of an organization, a social media user, or any other outlet, you assume responsibility to protect yourself against malicious scammers. I have listed some commonalities to help you identify common scams, and to help you protect yourself. I also strongly encourage members to follow three steps:

    1.Never list your email address or private telephone number on a website that is open to the public—this includes the ISST website, and your personal website

    2.Check your privacy settings on your social media accounts (Facebook, Twitter, Instragram, Google+)  to ensure that personal information is not open to the public. For instance, on Facebook, go into your "settings" and mark your page and postings so that they are only visible to “friends." Never accept someone as a "friend" who you do not know personally

    3.If you have a personal website that you have linked to the old ISST website, please update it with the new link: (www.SchemaTherapySociety.org)

    How to spot "Phishing" emails:

    1. The email has improper spelling or grammar: this is the most common sign of a "phishing" email, and the phishing email targeting our members was no exception. 

    2. The hyperlinked URL is different from the one shown in the email: when you hover your mouse over a link in the email (without clicking it), you may see that the actual URL differs from the one displayed. You can also hover your mouse over the email address in the 'from' field to see if the website domain matches the organization the email is supposed to have been sent from. 

    3. The email urges you to take immediate action: instead of following the action from the email, contact the person or organization directly through a verified contact number to see if the email is legitimate. Remember, it is highly unlikely that a member would reach out in the "stranded traveler" scenario.

    4. The email requests your personal information: reputable organizations don't ask for your personal information via email.

    5. The email says you've won a contest you haven't entered: claiming that you've won a lottery or a prize is a common phishing scam.

    6. The email asks you to make a dontation or help another organization member in need: scam artists often send out phishing emails inviting recipients to send money to help another member who is supposedly "in need" due to a crisis, and use the names of other members to help support their claim. Therapists are often targets of such "soft" phishing scams, and several other therapy organizations have been targeted with nearly identical messages.

    7. The email includes suspicious attachments: if you receive an email that looks in any way suspicious, never click to download the attachment, as it could be malware that may lead to phishing on your device--a much more serious threat that was not involved in this scam.

    What to do if you receive a suspicious email?

    1. If it involves ISST members or matters, please contact us immediately: a key to stopping the scammers in this latest incident was that other members quickly contacted us, which allowed us to warn all members we could contact. EMAIL: POTENTIAL SCAM

    2. Adjust your email settings for what is allowed in your inbox: use your email program to select incoming emails to be sent immediately to "junk," except those from your contacts and safe senders. You can then browse your "junk" folder to see which messages are legitimate. 

    3. Security software is a must: antivirus protection software can help warn you if a website is a forgery, and can stop malware from being activated on your device. 

    4. Report the phishing scams: major email providers offer tips to avoid scams, and email addresses to forward suspicious emails to.

    Your local or national government may have contact emails to report scams. Do a Google search to locate your local email address to forward the email to.

    DOWNLOAD THE INSPIRE 2016 APP NOW: Since you have registered for the Vienna conference, we want you to take advantage of the new Schema Therapy App, highlighting all the events at the Vienna World Conference.

    The app will still receive a few minor adjustments before the conference begins, such as the addition of more slides. If you are a presenter and see any information that needs to be modified, please contact us immediately. We will do the best we can to make the changes: EMAIL

     

    EARTH FRIENDLY: To help save trees, we will not be providing paper booklets or brochures at the conference, but instead will be relying on the conference app to give attendees all the information needed. We will also have the calendar of events posted on monitors within the venue. 

    Take a few minutes before you arrive in Vienna to download the app and choose your favorite events in the "Favorites" section of the app. You can then easily refer to your top selections. Each selection includes slides, if the presenter provided them.

    The calendar is set for Central European Daylight Time, so if you are in a different time zone, the app's event times will be off. When you arrive in Vienna, if your device does not automatically change time zones, you will need to change the time zone manually in your device's "Settings."

    FREE WIFI for the Schema Therapy App:  We have arranged for free wifi for conference attendees at the Messe Wien venue, so you will not have to use your data to access the app. The Schema Therapy App is now available in the Apple Store and in Google Play, and will work on any smartphone, tablet, or laptop, and will simplify your experience.

    Steps to Download the App

    1. Download the app  

    • iOS devices: use your iOS device (iPhone, iPad, etc.) to download the conference app. Click the image on the left, or go to: APPLE DEVICES
    • Android devices: use your smartphone or tablet to download the conference app. Click the image on the left, or go to: ANDROID
    • Others: if you use a Windows smartphone/tablet, or prefer to use your laptop (Mac or Windows), you can use your webbrowser to login to the app. Go to: BROWSER LINK

    2. Enter your information 

    Your username is: provided in a private email to registrants

    Your password is: provided in a private email to registrants

    Please use the email address this email is addressed to as your username.

    3. Within the app, click: "My Profile" 

    • Click "Edit My Profile" button  
    • Click "Change My Picture," and upload your favorite profile photograph   
    • Enter your first and last name (you can choose the name that you want to use) 
    • Click the "Facebook" and "Twitter" icons, and enter your usernames
    • Enter your home location (e.g. Stockholm, Sweden), and a brief personal description
    • Press "Save," located in the upper right corner
    • Click the "Menu" icon in the upper left corner to return to the main menu
    4. Reset Your Password: Go to "My Profile" menu, click the "Wheel"  in the upper right corner for the "Settings" menu
    • Under your username (your email address), type your temporary password.
    • Next, under your temporary password, create a new password that you will remember
    • Retype your new password 
    • Important: if you do not want other conference attendees to have the ability to chat with you, turn the chat button "Off." The default setting is "On"
    • Press "Save"
    5. Press the "Menu" icon on the upper left side. 

    6. Want kilometers? Click the "Settings" wheel in the main menu. Under "Distances," click "kilometers." The default is "miles." 

    7. Time Zone: the app is set for Central European Daylight Time. If your device does not automatically change time zones, please go into your device's settings menu to change the time zone when you arrive in Vienna so that calendar events appear at the correct local time for Vienna.

    Basic set-up is complete!

    Favorites: Personalize Your Experience

     

    You can easily personalize your experience through the "Favorites" feature. Mark every event or selection you want to attend as a "Favorite" so that they all appear within the "Favorite" icon in the main menu. For instance, you may want to include the morning yoga activity, a keynote address, your top choice for the morning selection, and your preferred lunch venue (all located within the app).  

     

    1. After choosing a selection, click the 3 dots in the upper right corner 

     

     

    2. Click the "Star" icon, and your selection will appear in your "Favorites"  

     

      

    3. Click the 3 dots in the upper right corner to return to the selection

     

    Comments within a Selection: when you are attending a presentation or event, you can post comments specific to the selection for either the presenter or fellow participants to reply to.  

    1. After choosing a selection, click the "comment" icon on the upper right corner of the app.

    2. Type your comment, then press "send"

    3. Everyone who has the app open on that particular selection will see the comment, and may reply to you. Keep in mind this is not a private "chat," but open to everyone who has access to the app. 

    Slides and Handouts: each selection or presentation may have slides and/or handouts available for attendees.  

    1. Within the selection, scroll to the top of the text to see "Link to Slides: Click Here." Most of the slides load quickly with a standard connection, however a few are extra large and may require a few moments to load. 

    2. To download, save, or print the slides or handouts before the conference, please click the appropriate icons within the slide or handout.

    You can view the entire calendar of events, or focus on a specific keynote, selection, or presenter, by clicking on the specific icon. Please note that events are set for Austrian time zone (Central European Daylight Time)

    Want to disable "Chat?" Click "My Profile" in the main menu, then click the "Wheel" in the upper right corner for "Settings." Under "Contact Rules," switch the lever to "Off."

    June 2016: Experiential Elements in Schema Therapy

     

    Schema Therapy relies on Cognitive, Behavioral and Experiential Strategies as well as Limited Reparenting.  In this issue we are pleased to highlight some new and creative approaches to Experiential work.

    Ofer Peled proposes eight different presentations of dysfunctional parent modes, and has a developed a hierarchy of confrontational interventions to address each presentation. Identifying both the the characteristics and the intensity of each dysfunctional parent mode, and understanding the patients unmet needs enables therapists to assume a stance in imagery or in mode work using chairs that will be most effective in helping patients get core needs met in a healthy way. 

    Christof Loose works with children and adolescents. His article describes a very creative technique he uses with children, in which he and the child identify the child’s modes, and write them down. The child then creates his own “mode sketch”, and assigns a finger puppet to each mode. Through the finger puppets the child is able to understand his modes, and work with them to find ways to get his needs met.

    Rita Yonan works extensively within a group schema therapy framework.  She looks at novel creative and experiential opportunities that are available within group schema therapy context. In addition, she discusses how experiential and emotional components of therapeutic work can be enhanced within a group format. 

    Chris Hayes is involved in research investigating the use of imagery respiting for childhood trauma. His article discusses imagery re-scripting within schema therapy context, and looks at practical ways therapists can improve imagery techniques and skills.

    Bruce Stevens and Pierre Cousineau contributed an article about Memory Reconsolidation and the brains ability to unlearn or relearn at the level of Emotional Learning. They describe recent findings which may help to explain why imagery is such a powerful intervention in Schema Therapy. 

    As these innovative techniques demonstrate, Schema Therapy remains a dynamic and evolving model. We look forward to continue to share our members creativity with you in future issues.

    CONFRONTATIONAL INTERVENTIONS

    A Hierarchy of Confrontational Interventions Facing 8 Dysfunctional Parent Modes

    Young et al. (2003) refer to the critical punitive and the demanding parent modes as negative internalized representations of parents and other significant figures. Since there are several parental styles (e.g. Maccoby & Martin, 1983) it is conceivable that there are much more internalized dysfunctional parent modes than the two suggested by Young. 

     

    Young et al. (2003) instruct therapists to help patients battle and defeat their internalized dysfunctional parent modes. Many patients and therapists find combating the parent voices makes them needlessly critical and punitive themselves. The hierarchy of confrontational interventions enables matching the intervention to the severity of the dysfunctional parent mode. As with real parental coaching, when the parent-child relationship is not that devastating, the hierarchy starts with empathic confrontation.  CONFRONTATIONAL INTERVENTIONS

    Let‘s imagine, there is a boy named Felix. His parents notice that something is wrong with him. His teacher says that he tends to be aggressive. They decide that he needs some help. They look for a therapist who can help Felix. Then, Felix gets to know Chris. He is a psychologist and therapist for children and adolescents. After Felix‘s parents have asked him to help, he meets up with Felix. Chris encourages Felix to talk about himself and his world.  He is especially interested in all the different sides of Felix that come out at different times. He calls these different sides „parts“ or „modes“. He asks him: Felix, I am really interested in finding out about you, and about all of your different sides, or „modes“ as I call them. Which different sides of yourself have you noticed? Let‘s write that down, okay?“ Felix creates his own mode-sketch. CHILDREN & ADOLESCENTS

    Experiential Strategies in Group Schema Therapy

    When I was asked to write this paper on “how to do experiential strategies in group” it got me thinking – what is different about implementing Schema Therapy experiential interventions in a group versus individual? This is something I do day to day, (for the last four years in fact) as part of my role as the Program Director of a Group Schema Therapy Program at a private psychiatric hospital in Melbourne, Australia (The Victoria Clinic). Although GST is theoretically consistent with individual ST and most individual interventions can be adapted to use in groups, a group does offer some additional creative opportunities. GST strategically uses the therapeutic factors of the group modality as described by Yalom. We also know from the Farrell, Shaw & Webber (2009) rct and the published pilot done in the Netherlands (Dickhaut & Arntz, 2014) that a group seems to catalyse the therapeutic effects of ST for patients with BPD. For this article, I discussed the issue with Joan Farrell and Ida Shaw, the developers of Group Schema Therapy for Borderline Personality Disorder and the creative minds behind the imaginative ways in delivering emotion focused techniques within the group setting. GROUP EXPERIENTIAL TECHNIQUES

    In over 25 (Bruce) and 40 (Pierre) years of being  therapists, we have sat for countless hours with patients who have struggled with accepted ‘truths’ about themselves, beliefs that have governed their lives, limited their choices and flooded their relationships with ugly emotions. We have agonized with our patients in the slowness of the change process. 

     

    One of the most important questions in therapy is how can the brain unlearn something? Think of one of your patients with early abuse or neglect. Or trauma. Or low self-esteem. Or dysfunctional relational patterns. This is often the focus of therapeutic efforts and it is frustrating how much the brain resists any change, even of early learning which is inherently irrational and completely dysfunctional.     “How about we do some imagery work today?” I said to my client, expecting a predictable imagery rescript where I enter the image, stick up and protect the child, and provide the safety that they had long been deprived. My supervisor at the time had been encouraging me to “do more experiential work." It had worked well in other clients and I was up for the challenge. 

     

    “Close your eyes and get an image of you as a child when you felt unsafe”, I said to my client. My client replied, “I’m walking into my house, I’m about 5, and as I walk in there are five big bikers who are drunk sitting in my lounge room…. I’m scared."

     

    Taken by surprise, I go through the imagery process trying my best to look calm and collected, but secretly thinking….“WHAT ON EARTH AM I GOING TO DO WITH FIVE BIKERS?!”. I looked around the room as if I was looking for an invisible guiding colleague to appear! IMAGERY RESCRIPTING

    What is AcroYoga? A team activity that combines three main elements:

    1. Solar Acrobatic Practices: cultivate trust, empowerment, and joy

    2. Lunar Healing Arts Practices: cultivate listening, loving, and letting go 

    3. Yogic Practices: cultivate breath awareness, life balance, and connection

    Step into your power and bring out your "Happy Child" mode: having fun, without knowing what you are capable of, and being curious about exploring new things.

    AcroYoga can teach you how to achieve through teamwork and progressions you may not have known were possible. You can laugh, play, and fall safely into a new sense of how much is possible in your body. As you advance in the session, you will become more skilled with how to fly, to be comfortably upside down, and how to support your fellow participants in the process of learning. Together, you become a team player, building trust through movements that are fun!

    The aim of this workshop is to establish and to improve a better and more profound understanding of Schema Therapy for Children and Adolescents (ST-CA). Starting with a breif overview of the model applied to children and adolescents, the workshop will cover working with drawings, finger puppets, chair work, board games, storytelling, fairy-tale techniques, marionettes, sand play, and metaphors. A significant part of the workshop will focus on the "Inner House," with a demonstration for how to use the technique, along with an exercise for participants.

    Imagery with rescripting is one of the most powerful experiential tools to meet and heal the Vulnerable Child mode, and to strengthen the Healthy Adult mode. In this workshop, you will learn how to apply rescripting throughout multiple disciplines. You will also learn how to use 'nonverbal' therapy (psychomotor, drama, and art therapy), and sociotherapy during rescripting. Finally, challenging situations that arise during rescripting will be discussed, such as how certain modes interfere with the process, and you will be able to practice working through these obstacles.

    Your logical starting point to explore the past and present:the Gothic Stephansdom Cathedral is Vienna's most beloved landmark, and remains the nucleus upon which the city has grown. The foundations of the original Romanesque church originate from 1147, but the earliest surviving features today are the 13th-century Giant's Door (Riesentor), and the Heathen Towers (Heidentürme). In the 14th and 15th centuries, various Habsburg rulers rebuilt the Gothic nave, the side chapels, and the choir.
